Indications/Uses
Adjunctive therapy in partial seizures w/ & w/o secondary generalization in adults & childn ≥6 yr & as monotherapy in partial seizures w/ or w/o secondary generalization in adults & adolescents ≥12 yr. Peripheral neuropathic pain eg, painful diabetic neuropathy & post-herpetic neuralgia in adults.
Dosage/Direction for Use
Adult & childn ≥12 yr Initial titration: Day 1: 300 mg once a day. Day 2: 300 mg bd. Day 3: 300 mg tds. Epilepsy Adult & adolescent 900-3,600 mg/day. May be initiated by titrating the dose (see Initial titration) or by administering 300 mg tds on Day 1. Based on individual patient response & tolerability, the dose can be further increased in 300 mg/day increments every 2-3 days up to a max dose of 3,600 mg/day. Slower titration may be appropriate for individual patients. The min time to reach a dose of 1,800 mg/day is 1 wk, to reach 2,400 mg/day is a total of 2 wk, & to reach 3,600 mg/day is a total of 3 wk. Dosages up to 4,800 mg/day have been well tolerated. Childn ≥6 yr Starting dose: 10-15 mg/kg/day. Effective dose: 25-35 mg/kg/day, reached by upward titration over a period of approx 3 days. Dosages up to 50 mg/kg/day have been well tolerated. Peripheral neuropathic pain Adult May be initiated by titrating the dose (see Initial titration). Alternatively, starting dose is 900 mg/day given as 3 equally divided doses. Based on individual patient response & tolerability, the dose can be further increased in 300 mg/day increments every 2-3 days up to a max dose of 3,600 mg/day. Slower titration may be appropriate for individual patients. The min time to reach a dose of 1,800 mg/day is 1 wk, to reach 2,400 mg/day is a total of 2 wk, & to reach 3,600 mg/day is a total of 3 wk.
Administration
May be taken with or without food.
Special Precautions
Status epilepticus; renal insufficiency; suicidality.
Adverse Reactions
Peripheral edema, myalgia, fatigue. Neurologic & psychiatric disorders.
Drug Interactions
Basic Al carbonate, Al-hydroxide, -phosphate; dihydroxyaluminium-aminoacetate, -Na carbonate; magaldrate; Mg-carbonate, -hydroxide, -oxide, -trisilicate; ginkgo.
